Generic Drugs Overview

As stated by the Food and Drug Administration (FDA) - a generic drug is
identical, or bioequivalent to a brand name drug in dosage form,
safety, strength, route of administration, quality, performance
characteristics and intended use. In other words,
pharmacological effects of generic drugs are exactly the same as
those of their brand name counterparts. Generic version of the
drug has the same active ingredient with the same chemical
purity as the brand name drug. However, trademark laws in the
United States do not allow the generic drug to look exactly like
the brand name drug. So other ingredients such as tablet
fillers, binders, coatings, flavors, or colors usually are
different.

The major difference between brand name and generic drug is the
research process required for the brand name company to
discover, design, and develop the new drug. This process
includes preclinical testing of a new drug in laboratories,
several phases of clinical studies in volunteers and people who
have the disorder being studied and finally FDA review and
approval. The whole process usually takes about 10 years and on
average can cost a drug company about $500 million.

The FDA grants the innovator company a patent that gives the
company an exclusive right to a drug for 20 years. Additional
patents can sometimes be filed to extend the patent life. After
a patent has expired, other companies are allowed to manufacture
and sell a generic version of the drug.

For the healthcare industry, generic drugs offer significant
savings to consumers. According to the Congressional Budget
Office, generic drugs save consumers an estimated $8 to $10
billion a year at retail pharmacies. Potential savings are even
greater. American consumers could have saved $20 billion in
2004, and even more could be saved in 2005 and future years,
says a report released by Express Scripts Inc.

Generic drugs are less expensive because generic manufacturers
don’t have the investment costs of the developer of a new drug
and so they can sell their product at substantial discounts.
Also, when multiple companies begin producing and selling
generic versions of the same brand name drug, the competition
among them also drives the price down even further. Today,
almost half of all prescriptions are filled with generic drugs.

Before generic drugs can be marketed, they must be approved by
the FDA. To start approval process, generic company has to
submit Abbreviated New Drug Application (ANDA) to the FDA.

The main stages of the generic drug review process are the
following:

Bioequivalence Review - this process establishes that the
proposed generic drug is bioequivalent (within a few percentage
points) to the brand name drug. Generic drug is bioequivalent to
brand name drug if both the rate and extent of absorption of the
active ingredient of the generic drug fall within established
parameters when compared to that of the brand name drug.
Bioequivalence of different versions of a drug can vary by up to
20%, because for most drugs, such variations do not noticeably
alter effectiveness or safety. However, actual differences
between FDA approved generics and brand name drugs are typically
only about 3.5% on average and rarely exceed 10%.

Chemistry/Microbiology review - this process provides
assurance that the generic drug will be manufactured in a
reproducible manner under controlled conditions. Among areas
that are subject to check are manufacturing procedures, raw
material specifications and controls, sterilization process,
container and closure systems.

Request for Plant Inspection - Upon filing an ANDA an
establishment evaluation request is forwarded to the Office of
Compliance to determine whether or not every link in the drug
production chain is operating in compliance with current Good
Manufacturing Practice regulations. Each facility listed on the
evaluation request is evaluated individually and the Office of
Compliance makes an overall evaluation for the entire
application.

Labeling review - this process ensures that the proposed
generic drug labeling (package insert, container, package label
and patient information) is identical to that of the brand name
drug except for differences due to changes in the manufacturer,
distributor, pending exclusivity issues, or other
characteristics inherent to the generic drug.

Based on the results of bioequivalence review generic drug
receives a two-letter therapeutic equivalence evaluations code.
The coding system for therapeutic equivalence evaluations is
constructed to allow users to determine whether the FDA has
evaluated a particular approved product as therapeutically
equivalent to other products (first letter) and to provide
additional information on the basis of evaluations (second
letter).

Drugs are considered to be therapeutically equivalents only if
they contain the same active ingredients, are of the same route
of administration and are identical in strength or
concentration, and if they can be expected to have the same
clinical effect and safety profile.

The two basic categories into which drugs have been placed are
indicated by the first letter as follows:

A - drug that FDA considers to be therapeutically
equivalent to other drugs. If there are no known or suspected
bioequivalence problems, the generic drug receives one of the
following codes:

AA - Products in conventional dosage forms not presenting
bioequivalence problems

AN - Solutions and powders for aerosolization

AO - Injectable oil solutions

AP - Injectable aqueous solutions

AT - Topical products (creams, gels, lotions, oils,
ointments, pastes, solutions, sprays and suppositories)

AB - If actual or potential bioequivalence problems have
been resolved with adequate evidence supporting bioequivalence

B - drug that FDA, at this time, considers not to be
therapeutically equivalent other pharmaceutically equivalent
products:

BC - Extended-release dosage forms (capsules, injectables
and tablets)

BD - Active ingredients and dosage forms with documented
bioequivalence problems

BE - Delayed-release oral dosage forms

BN - Products in aerosol-nebulizer drug delivery systems

BP - Active ingredients and dosage forms with potential
bioequivalence problems

BR - Suppositories or enemas that deliver drugs for
systemic absorption

BS - Products having drug standard deficiencies

BT - Topical products with bioequivalence issues

BX - Drugs for which the data are insufficient to
determine therapeutic equivalence

B* - Drugs requiring further FDA investigation and review
to determine therapeutic equivalence

If the generic drug is placed into B category this does not mean
that it is not good or is of lower quality. It simply means that
if you started using that generic, you should not switch to the
brand name or vice versa especially if that drug has a narrow
therapeutic range (some antidepressants, corticosteroid tablets,
antihypertensive drugs).

To ensure the quality of generic drugs, FDA inspects facilities
where drugs are manufactured about 3,500 times a year. Both
brand name and generic drug facilities must meet the same
standards of good manufacturing practices. FDA does not permit
drugs to be made in substandard facilities. Manufacturers of
generic drugs have facilities comparable to those of
manufacturers of brand name drugs. In fact, with the potential
to lose millions of dollars to a generic companies, some brand
name firms have either bought smaller generic companies or
created their own. FDA estimates that 50% of generic drugs are
produced is by brand name companies. They frequently make copies
of their own drugs but sell them under the generic name (e.g. GlaxoSmithKline and Pfizer produce generic
versions of their own drugs Paxil and Neurontin).

Not all brand name drugs have generic versions. Sometimes a drug
is too hard to duplicate, or there are no adequate tests to
prove that the generic drug acts the same as the brand name
drug. Sometimes the market for the drug is too small and there
is no business sense to create another version of a drug. You
can search for all generic equivalents of brand name drugs by
using the “FDA
Electronic Orange Book”.

A stiffer wakeboard rope is required for wakeboarding than for waterskiing. To help you to perform tricks you get more air when you use a tighter stiffer rope that does not stretch. You will be better able to pull yourself through flips and spins.

There are a number of types of wakeboard ropes, so, when shopping make sure to keep your specific needs in mind.

No stretch ropes use a material called Spectra in their construction. The Spectra rope has almost no elastic properties and is very strong. The stretch, when under load is less than half a percent.

Low-stretch generally stretches one percent in length under a normal wakeboarding load. These ropes are for the most part made of polyethylene or some blend of it. If wakeboarding is your primary interest, learning tricks and honing your skills, a stiffer rope is the best choice.

If you do both wakeboarding and waterskiing on a recreational basis a low-stretch rope will work well for you. It will have enough stretch for water skiing and enough stiffness to pull against when wakeboarding.

Wakeboard ropes come in various lengths but average between sixty to seventy feet. There are wakeboard ropes of one piece with no length adjustment and some that offer multiple adjustment loops.

The handles for wakeboarding tend to be more specialized so that making aerial maneuvers or doing tricks become easier. Wakeboard handles generally have a wider grip, thirteen to fifteen inches as compared to the eleven to thirteen inches usual for waterski handles. It is much easier to pass the wider handle behind your back when performing tricks of that nature.

Most wakeboard handles have a rope braid or a second, smaller handle grip built into the rope. Having another grip to hold on to gives the rider a stronger hold. Wakeboard handles usually have neoprene foam floats so they stay on top of the water.

Option Spreads - Credit and Debit Spread Trading

People who trade options often will engage in trading spreads. A spread is the buying and selling of the same type of option. A Call Option spread is buying and selling (writing) call options. A Put Option spread is buying and writing puts.
The purpose of engaging spread trading is to either make money on the premium difference (money spent and received) or to earn profit on the options themselves being traded or exercised.

Debit or Credit Spread

A debit spread is when the options that are bought and sold result in a loss on the premiums. The investor has spent more for the option purchased than the option shorted.

An example of this would be:

Long (buy) 1 ASD SEP 40 CALL@4 and Short (sell) 1 ASD SEP 45 CALL@2

This is a debit spread since the $400 paid exceeds the $200 received. There is a $200 Debit on this spread. The investor in this case is looking to make a profit on the future value of the options. Since these are call options, the investor is bullish on the market (wants the market on ASD to rise).

The market rising will allow the investor to take advantage of the increased premium or to exercise the options. The long option allows the investor to purchase the stock at 40 and the short option carries an obligation to sell at 45. If these were to happen, the person could make 5 points on the stock (strike price difference) minus the initial debit loss ($200). This equals the maximum gain potential ($300). The maximum loss is if both options expire worthless, resulting in a $200 loss.

A credit spread works the opposite way. The investor is looking to gain on the premiums and then is hoping the options expire worthless. Using the same example above, the numbers are the same, but the gain and loss would be reversed. The person would be Long the 45 paying $200 and Short the 40 call, gaining $400. The $200 is now a credit and is the gain. If the options were exercised, the 5 point difference in the strike prices would be a loss (buying at 45 and selling at 40). The trader would be bearish on the market for a call credit spread like this. Trading of credit call spreads is higher in a bear market.

Vertical Spread

A vertical or price spread is when the strike prices are different, but the expiration months are the same. The above examples would be considered vertical spreads.

Horizontal - Calendar Spread

A horizontal spread is when the strike prices are the same, but the expiration months are different. The trader can make money on this type of spread because even thought the strike prices are the same, the option with the longer expiration month will have a higher premium, so there is still a “spread”.

Diagonal spread

When a spread has months and strike prices that are different, it is defined as a diagonal spread. The options are vertical and horizontal at the same time.

All in all, spreads are fairly conservative - as far as options are concerned. A long position is covered by a short position, so large or unlimited losses do not normally occur.

When speaking to parents about child identification, the first things that come to mind are fingerprints and DNA information. However, there seems to be a complete lack of awareness regarding fingerprints and the crucial need in updating them on a regular basis.

Most parents, with the exception of people who have studied medicine or criminology, believe that their children’s fingerprints remain the same from birth to death.

We all believe our fingerprints to be as permanent as a tiger’s stripes since they are formed before our birth, while in the womb. Though this is absolutely correct, here is the kicker; children’s fingerprints are actually changing for the first five to seven years of their life.

The easiest way to explain this contradiction to you is with the following example; picture an under-inflated balloon with a picture on its surface, as this under-inflated balloon has air added to it, the picture becomes larger and becomes somewhat distorted.

With this in mind, think about the size of a newborn’s fingers. Pretty darn tiny!

On average it takes children approximately eighteen to twenty one months before their very tiny fingerprints have developed enough to be of any use. We have all heard the expression “As smooth as a baby’s bottom” this also applies to fingers!

That is why footprints are taken for children younger than twenty-one months of age.

Now getting back to our “balloon”, think of your child’s fingerprints as the picture on the balloon, as they grow older, their fingerprints, though they are actually changing, remain the same. One thing to keep in mind is that as your children grow older, their fingerprints might also change due to their skin’s flexibility and also due to disfiguration caused by a scar(s).

When studying fingerprints, the authorities use certain identifying features or characteristic points: ridge endings, dots and bifurcations, in order to make a positive identification. With every passing year of your child’s life, their fingers are growing in size, and these characteristic points become more pronounced, it becomes easier for the authorities to read your child’s fingerprints.

For this reason, it is your task, as a responsible parent to update your child’s fingerprints at least once a year. The thought of the fingerprints ever becoming useful is in itself a bone chilling one, for they are only used after the unimaginable has happened, passive identification. However, if needed, do you not want to provide the authorities with your child’s the most accurate and easiest to distinguish identification?

Our next tidbit of advice is on the location in which parents keep their children’s fingerprints. We recommend you keep them in a Ziploc baggie in the bottom of the freezer.

Here are some of the reasons for this suggestion:

- Your children do not play in the freezer and other than food nothing is kept in the freezer, therefore, you will always know exactly where they are.

- If you are not at home, you can easily direct a babysitter or neighbor to your freezer.

- Unlike a bank safety deposit box, you always have access to your freezer.

- Unlike a home safe or strong box, if in a state of panic, you don’t have to try and remember a four, five or six digit combination, or try to explain to a babysitter how to open your safe.

Our logic behind this suggestion is; should the unthinkable ever happen, the minute the authorities knock on your door, you want to have your child’s identification/fingerprint kit in their hands. Time is of the essence; you do not want to be tearing the house apart trying to remember where your child’s fingerprints are.

Last tidbit; when leaving town on holidays, don’t forget to pack your children’s identification kit. Once again if something ever happened, your kids identification will not be of much use, two thousand miles away in your freezer.

Our fingerprints are completely unique, one of a kind! Identical twins do not have the same fingerprints, although they do share the same DNA.

Magnets: How Do They Really Work?

Can magnets actually help us feel better? When I first read about this theory I was somewhat skeptical but curious. After looking into it further and trying out a few magnetic products, I was pleasantly surprised. Let me tell you a few things I found out.

Research has shown that when charged particles pass through a magnetic field, they generate a current, which produces heat, expands blood vessels, and increases blood flow. This in turn can stimulate the body to heal faster, as the movement of oxygen and other nutrients to the cells increases and unwanted waste and toxins are flushed from the system. Although the body will often heal in its own time, we can expedite nature’s clock by applying a magnetic field. Through the use of magnets we can aid the cell’s natural ability to heal itself by providing the optimal environment for that repair.

As described in Sherry Kahn’s book, Healing Magnets, magnet therapy relieves pain, speeds healing, and appears to restore balance when our systems get out of whack. While research is still somewhat limited, a variety of studies have been done in which magnetic therapy has been helpful for conditions such as back, neck, and shoulder pain, bone fractures, carpal tunnel syndrome, dental problems, depression, fatigue, female problems, fibromyalgia, foot pain (diabetic neuropathy), headaches, high blood pressure, insomnia, multiple sclerosis, osteoarthritis, Parkinson’s’s disease, postpolio syndrome, rheumatoid arthritis, skin problems, soft-tissue injuries, recovery from surgery, wound healing, etc.

One particular randomized, double-blind, placebo-controlled trial was conducted by Dr. Michael Weintraub and published in the May 5, 2003 issue of the Archives of Physical Medicine (a leading publication on rehabilitative medicine that is part of the American Medical Association family of scholarly journals) confirms that the constant wearing of static, permanent, magnetic insoles produces statistically significant reduction of neuropathic pain. Problem Solving - Think Cleopatra’s Ass

Picture the scene. Anthony pops down to the guardhouse, partly because he wants a break with the lads, but also because he has a problem. Cleopatra says she wants to bathe in ass’s milk. So Anthony tells the guys in the guardhouse, that he needs
some help. “The wife wants to bathe in ass’s milk now.” He says.

Gerald, the Head Guard, says, “She wants to what?” “Bathe in ass’s milk”, says Anthony, “You know what these women are like - she’s read it on some tablets of stone somewhere - it’s supposed to be good for her complexion”.

“So how are you going to sort that out then,” says Gerald, “Are you going to get someone to pour it over her or something?” Anthony thinks about it and says, “No, that won’t do, she wants to soak in it. She needs a ‘bath’.”

“What’s a ‘bath’ then..”, says the Gerald. “It’s a big thing you get into that’s full of the ass’s milk - and then you lie down in it,” says Anthony.

“What do you want to do that for then..?” asks one of the new guards, a young lad really.

“So you can get clean all over and relax in it - let the ass’s milk do it’s job.” Says Anthony, trying to sound as if it makes sense. “Oh, right then..” Says the Head Guard.

“Nah, I don’t know where you’ll get one of them.”

Anthony goes on his way, this time stopping at the stonemason’s yard, thinking all of a sudden, those big stones we shape, all we need to do is to hollow on out and that will do.

So he starts a conversation with the Head Stonemason, Steve.
“Can you do me a ‘bath’”, he says. “What’s one of them, then?”, the rather busy stonemason says. “It’s a big thing you get into and lie in, full of hot liquid. It’s relaxing.” Anthony says, sounding less and less convincing each time. “You can do it, with this stuff you’re cutting up each day, it’ll be a breeze”. “Nah, mate,” says Steve, “This stuff is sandstone, it won’t hold the liquid”.

Disappointed, but losing a bit of interest, Anthony moves along in his quest.

But it starts the stonemason thinking - he has a bit of spare granite out the back of the shed, so over the next few days, he does a bit if chiselling and sees what he can do.

A week the following Friday, he gives old Anthony a shout and shows him his work. “Brilliant”, says Anthony - let’s get it installed, she’ll be having her nails done all afternoon. “Let’s get those asses milked too”, he shouts to his PA.

Later that afternoon, Cleopatra has a big surprise and excitedly clambers into the new granite bath, filled with warm asses milk and she is in seventh heaven.

Bet you’re wondering what the moral of the story is eh?

Well, in our everyday job, we are faced with dozens of little challenges. New problems to puzzle out. And they cause us irritation and frustration. Yet, like the stone mason, Steve, we have people around us who, left to their own devices and interested enough in the challenge, will sort things out - in their own way and in their own time.

It’s as true today as it was when Anthony and Cleopatra were around.

There is a postscript to this. Two days later the stonemason is called up to the new ‘bath’ room. Cleopatra was pleased to see him and thanked him for his excellent work but asked, discreetly, if he could bring his polishing stone up with him
later.

“The bath is great, but I have a little problem. There’s something right in the bottom corner that is a little rough - in fact, she said, it’s a bit of a pain in the ass.”

Knowingly, the stonemason left the bathroom for his tools. “I know, I know” he said, “We get them all the time.” And he smiled a little smile as he prepared to meet a challenge of a more delicate nature.
